HRSN Social Care Network Coordinator (NYC)

Fountain House is hiring a full-time HRSN (Health Related Social Needs) Social Care Network Coordinator.

Responsibilities include:

• Complete extensive training on Social Care Networks and Health-Related Social Needs regarding screening, navigating, providing HRSN services, and documentation requirements on designated data platforms such as Unite Us and Find Help.

• Assist in creating systems to identify individuals’ Medicaid status, screen all eligible members, navigate members to Health-Related Social Needs both to Fountain House services as well as to outside organizations participating in the Social Care Network.

• Ensure all HRSN screening and navigation conducted both in person and over the phone is completed in a timely manner adhering to strict deadlines.

• Document all engagements in platforms as designated and required by the larger Social Care Network.
